An exciting opportunity has arisen in Adult Social Care (ASC). We are looking for a qualified Social Worker, with experience of working with adults and young people (including those with learning disabilities and people with impairments), to join our supportive and highly skilled service.
You will be required to review cases involving vulnerable people that have a care package or service via Direct Payments, to ensure that they are safe, protected from abuse and neglect as set out by the Care Act 2014.
Your reviews will focus on high-cost care packages, seeking alternative funding streams including eligibility of packages for Continuing Health Care.
To complete a follow up re-assessment of cases that appear to show evidence of unnecessarily high costs.
Post completion of the Direct Payments review, to add resource to our focus upon timely reviews of high-cost placements.
To focus on specific high-cost placements where a review has not been undertaken for some time, with a view to seeing what scope exists to reduce costs.
To provide high quality, professional service to all adults/older people who require support. To carry out strength-based reviews that are compliant with Care Act 2014.
To use skills and professional knowledge to ensure people remain safe and well in our communities.

You will need to be an excellent communicator and able to work alongside providers, colleagues in Health and Voluntary organisations.
