Position Title: RN Triage

Department: Primary Care

 

 

QUALIFICATIONS:


 REQUIRED:

 Graduate of an accredited school of nursing

Current RN licensure in Minnesota

Current RN licensure in Wisconsin within two months of hire

Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable

Strongly demonstrated written and oral communication skills

 

PREFERRED:

 One year full time employment as an RN

 

PHYSICAL REQUIREMENTS:

Ability to sit or stand for prolonged periods at a time. Must possess speech, hearing and sight skills to perform the functions of the position. Manual dexterity required. Must be able to speak, read and write English proficiently. Proficiency in other languages may be required. Some positions may have lifting requirements.


 POSITION PURPOSE:

Service excellence is to be centered on patient care and patient relationships and is the responsibility of all employees.  Teamwork is the norm and all employees will be held accountable to work as effective multidisciplinary care team members.

Effectively provides high quality service to both internal and external customers

Provides comprehensive assessments to establish and evaluate a plan of care and address changes in a patient’s condition.

Delegates nursing tasks when appropriate and provides supervision and monitoring of function or activity

Creates and implements dynamic teaching plans based upon identified patient need

Acts as a resource for other staff

 Nursing Process:

Definition

Nursing assessment is accomplished through interviewing, observation, and/or nursing procedures.

Problem identification defines the impact of health problems affecting individuals and families. Registered nurses distinguish between normal and abnormal findings in order to identify patient capabilities and limitations.

Planning is the process of making decisions and setting goals which are mutually acceptable to the nurse and all pertinent others.

Implementation is the process of providing comprehensive nursing care.

Evaluation includes assessment and reassessment of the patient’s progress toward goal achievement.

Performance is up to standard when:

Nursing Care:

Demonstrates clinical competence in using the nursing process to provide care to patients and their families.

Implements established Nursing Policies and Procedures and Standing Orders.

Provides emotional, intellectual and psychological support to patients and their families.

Documentation:

Documents accurately and completely and keeps record regarding clinical practice.

Consultation / Collaboration:

Shares information with other members of the health care team.

Makes appropriate patient referrals.

Uses interdisciplinary resources to enhance own clinical decision making

Provides continuity among home, office, inpatient setting and community.

Uses appropriate systems, automated or manual, to obtain patient information or schedule appointments.

 

Care Coordination:

Definition:

Care Coordination is the ongoing support of patients and their families over time as they navigate care and relationships. It assists patients/families to optimally manage their health care, respect their culture and values and helps to identify individual needs, health goals and treatment preferences.

Performance is up to standard when:

Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans

Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care

Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention

Identifies and secures patient services needed for patient care

Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.


Education:

 Definition:

Education imparts knowledge designed to develop skills, to enhance decision-making, and to promote understanding. It includes interaction with the patient, family and community, as well as participating in formal and informal professional education.

Performance is up to standard when:

Patient / Family / Community:

Works in partnership with patient/family to identify patient and family needs and develop a plan for education and/or make appropriate nursing referrals.

Teaches techniques or knowledge necessary for prevention, self-care management or rehabilitation.

Engages in ongoing evaluation of the effectiveness of teaching in terms of understanding and application.

Professional:

Participates in patient care conferences, in-services and educational programs.

Keeps current on best practices related to nursing and patient care

Leadership:

Definition:

Nursing leadership is the possession of skills and knowledge to manage situations, assess problems and facilitate solutions. It includes listening, decision-making, delegating, communication, motivating, facilitating, role modeling, critical thinking and problem solving.

Performance is up to standard when:

Role Modeling:

Serves as a role model in the enhancements of nursing practice and professional conduct.

Demonstrates flexibility in performing additional responsibilities to contribute to the improvements in patient care and the functioning of the care unit and other support services.

Provides support to professional and technical staff.

Goal Determination / Achievement:

Formulates and attains personal and professional goals which promote individual growth, and support the organization vision, mission and values.

Decision-Making / Problem-Solving:

Demonstrates ability to assess and problem solve.

Serves as a catalyst for improving standards of practice.

Communication Skills:

Demonstrates positive verbal and written communication and effective listening skills with internal and external customers.

Delegation:

Delegates appropriately and holds self and others accountable for safe delivery of the delegated care.

 

Professionalism:

A. Definition:

Professionalism includes knowledge, self-direction, accountability and the ability to use sound critical judgment in decision-making.

Patient Advocacy:

Acts as a patient advocate

Maintains patient confidentiality

Provides current health education about diagnostic and prescribed treatments

Professional Growth:

Selects and participates in appropriate professional conferences, workshops and continuing education and committees.

Research appropriate to clinical setting:

Participates in and utilizes research appropriate to clinical setting to improve patient care.

Utilization of Resources:

Remains accountable for the efficient use of time, material and human resources.

Assists with the updating of current and development of new procedures as appropriate.

Standards:

Adheres to nursing standards of conduct, performance and ethics.
