You will be working as an autonomous practitioner within the Integrated Respiratory Services in North Mid in the Community. You will be flexible and work across a geographical area.
You will provide specialist respiratory assessment, diagnosis and evidence-based treatment programmes to meet a wide variety of patient needs in the most appropriate location in the community. You will also provide advice, guidance and information to health and social care professionals, carers and relatives, with regards to respiratory care. This will include Pulmonary Rehabilitation, Chest Clearance Clinics and Home visits.
You will be responsible for organising and providing pulmonary rehabilitation to the patients of Enfield in the community setting to the highest standard with the respiratory assistant.
You will act as a source of physiotherapy advice for other members of the team which includes the primary care team, other health professionals, acute... services, social services, voluntary sector, private sector homes, patients and their carers.
Main duties of the job
To be clinically responsible for a designated caseload of respiratory patients including COPD and rehabilitation patients, ensuring high standards of care.
Organise workload effectively and efficiently with regard to clinical priorities and use of time, and in accordance with service demand/ requirements on a changing basis.
Use of relevant and appropriate screening tools.
To adhere to recent evidence base and review patients with a view to ensure safe and effective COPD treatments/ management.
Act as an educational resource for patients, families/ carers and other healthcare professional/ multidisciplinary colleagues, with respect to all aspects of COPD and rehabilitation.
To contribute to the planning and evaluation of community based Pulmonary Rehabilitation programmes.
To deliver Pulmonary Rehabilitation programmes acting as an autonomous practitioner.
To provide advanced physiotherapeutic assessment of patients with diverse presentations and complex physical, psychological and emotional conditions.
To use advanced clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition.

Person Specification
Skills and abilities
• Minimum 3 years NHS post registration experience
• Experience of coordinating and managing patient care
• Previous experience of delivering specialist respiratory / rehabilitation care
• Able to take a self-reflective approach to own practice and to engage in clinical supervision.
• Able to diffuse potential conflict or aggressive situations
• Awareness of self as a role model for junior staff
Other essential requirements
• Effective time management
• Car owner with full UK licence
Experience and knowledge
• Excellent verbal and written communication skills
• Knowledge of management of change, research, audit quality
• Knowledge of research and evidence base within the field of rehabilitation, COPD and Heart Failure
• Effective teaching and training skills
Education and qualifications
• Degree in Physiotherapy
• Evidence of CPD maintained in a portfolio including attendance at recent in service training in respiratory
• State Registration
