12 months of caregiving experience (required... (Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice)
• Hospice experience (preferred)
• Ability to work in an electronic medical record system
• Valid driver's license (required)

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
• Review patient care plans
• Assist with dining and nutrition
• Maintain electronic medical records
• Company car with fuel and insurance covered
• Company cell phone
